An Inpatient Feedback survey form is given to patients to complete prior to discharge. Patients are provided with a Reply Paid envelope addressed to Service Improvement Team, Roma. The Inpatient Feedback survey can be mailed, handed to staff on duty or placed in the locked box if provided in the ward.
